I would recommend finding another doctor, this office violates the law and your rights.

Stay away if you want to stay legal and not have a bunch of advertisements sent to you from your personal contact info contained in your medical marijuana paperwork (also known as a hippa violation you cant take a number from a medical record for advertising purposes)
